SIGNIFICANCE OF COLOUR DOPPLER STUDY IN PREGNANCY INDUCED HYPERTENSION (PIH) AND ITS CORRELATION WITH PERINATAL OUTCOME

Background: We found that colour Doppler is an excellent tool for non-invasive hemodynamic monitoring of PIH patients. It helps to identify the fetus at risk & predict perinatal morbidity & mortality. Doppler velocimetry can guide us in the treatment of these pregnancies & prevention of high mortality & morbidity in hypertensive patients.

Methods: This is a Prospective longitudinal study. In our study, 125 patients participated among them 67 Mild Hypertensive 41 Moderate hypertensive and 17 severe hypertensive patients We did a study to evaluate the role of doppler ultrasonography in predicting the adverse perinatal outcome in high risk PIH pregnancies using uterine, umbilical and middle cerebral artery doppler indices.

Results: We found that patients have higher LSCS rate (50.48%). PIH was associated with IUGR (36.19%) LBW (46.67%), poor 5min APGAR score (32.38%) and SNCU admission with (35.24%). We found relatively higher rate of fetal distress & meconium-stained liquor (29.52%) was associated with higher perianal mortality (17.65%). Doppler ultrasonography is predicting the adverse perinatal outcome in high-risk pregnancies using uterine, umbilical and middle cerebral artery Doppler indices.

Conclusions: In our study we found abnormal colour Doppler indices are very important predictor for fetal outcome. There should be regular doppler assessment Sono graphically. In high-risk cases colour Doppler study should be evaluated at regular interval.
