Primary amenorrhea is the absence of menstruation by 15 years of age in the presence of normal secondary sexual characters. Cervical Hypoplasia is a rare congenital disorder. 16yr old presented with the complaint of cyclical pain abdomen and absence of menstruation. On examination normal secondary sexual characters present. MRI revealed narrowing of vaginal canal with grossly distended endometrial and endocervical canal with hemorrhagic contents. History of reconstructive pelvic surgery around 1yr back given by the patient. Hence decision of laparotomy taken. Intra-operative findings-uterus 12 wks. soft, a rudimentary horn attached to main uterine body on the right side noted, B/L tubes and ovaries apparently healthy. Hematometra drained by a 2 cm vertical low anterior incision on the body of the uterus. Cervical Hypoplasia noted with no communication. Cervical agenesis or hypoplasia is a distressing diagnosis for the patient and their family, often due to a focus on the inability of the patient to have a normal menstrual cycle and subsequently carry a pregnancy.
